Bug 15576: Fix for incorrect link redirect

To test:

1) Edit a user to have an uncertain address (gone no address)
2) Log in as that user on OPAC
3) Click 'online update form' link in the yellow message
4) Confirm you are taken to opac-memberentry.pl
