Ran into BUG() while doing madvise(REMOVE) testing.  If we are punching a
hole into shared memory segment using madvise(REMOVE) and the entire hole
is below the indirect blocks, we hit following assert.

        BUG_ON(limit <= SHMEM_NR_DIRECT);
